For self-pay: inform us of the therapist you wish to speak with.
For insurance benefits: send us a copy of the front and back of your card along with your date of birth.
For EAP benefits: send us the authorization number and the EAP managment company name.
Once the financial aspects have been properly agreed upon provide us with either a time of day or day of the week that best works with your schedule and we will do our best to to accommodate your need.
We use a web-based Electronic Health Record (EHR). All scheduling is completed in the EHR and allows clients to see upcoming and past appointments, as well as, make any payments via card processing. To sign up with our agency you are required to establish a client profile. The set up for a client profile takes just a few minutes and is completed via an email link that is created after discussing scheduling options. The link leads you to a document that requests demographic information and several electronic signatures.
